Supporting Information:

We will ask you to provide some documents to support your application.

Please look out for an e-mail from:

applications@caudwellchildren.com

and check your spam and junk-mail folders.

This may take up to 4 weeks depending on the number of people applying.

Eligibility

We will review the information you send us to make sure that the pack is suitable for your child.

Once we have received your supporting documents your application will be assessed.

This is usually completed within 28 days but may take longer depending on demand.

Application Outcome

If your application is successful, we will send you a Donation Agreement.

You need to complete this and return it to us with your £20.00 contribution.

Once we have received your Donation Agreement and £20.00 we will order your pack.

Delivery of your pack should then take about 21 days.

If your application is unsuccessful we will send you a decline letter

We’re really keen to understand the difference Get Sensory Packs are making to children.

The information you provide to us as part of your application and once you have received your pack will help us to support more families in the future.

For this reason, we will ask you to complete a questionnaire three times within the next year to understand how things are and whether they have changed for you as a result of receiving support from us.

If you would like help, advice or support with any of the issues identified through this questionnaire then please contact: familyservices@caudwellchildren.com
